to let us know who sent them when they call us for an appointment, and
we will keep a
record. In
order for you
to always know how many Referral Rewards you have accumulated, you can
scroll
down to the bottom of this page to find the list we have created with
your first name
and last name
initial. Along with your name,
you will see
the first name and last name initial of the people you refer to us.
To
benefit from this fun program, please remember the following...
